How It Works

Step 1: Initial Nursing Assessment
A Registered Nurse meets with you (and your family/carers) to assess your health needs, review medical history, medications, and understand your goals.

Step 2: Nursing Care Plan Development
Your RN develops a tailored nursing care plan in collaboration with your GP, specialists, allied health team, and family.

Step 3: Ongoing Nursing Visits
Your assigned RN provides regular home visits to deliver clinical care, monitor your health, adjust your plan, and coordinate with your medical team.

Step 4: Communication & Coordination
We maintain open communication with your GP, specialists, support workers, and family—ensuring everyone stays informed and aligned.

 Not necessarily, but your NDIS plan must include funding for nursing supports. Your GP or specialist may provide documentation to support your funding request during plan reviews.

 Community nursing involves clinical tasks performed by Registered Nurses (medication, wound care, injections, clinical assessment). Personal care involves non-clinical tasks performed by support workers (showering, dressing, meal prep).

Yes. Our RNs provide training, delegation, and ongoing oversight for support workers delivering certain high-intensity care tasks under nursing supervision.

 It depends on your health needs and NDIS plan. Some participants need daily visits; others need weekly, fortnightly, or monthly check-ins.

 NDIS community nursing is funded through your NDIS plan, not Medicare. However, GP visits and hospital care remain covered by Medicare.
