Support Coordination Levels
The NDIS funds three levels of Support Coordination, depending on the complexity of your needs:
Level 1: Support Connection
Short-term assistance to connect with providers and understand your plan. Suitable for participants with straightforward needs and lower complexity.
Level 2: Support Coordination (Most Common)
Ongoing coordination for participants with moderate complexity or multiple providers. Includes regular check-ins, provider connections, and capacity building.
Level 3: Specialist Support Coordination
Intensive coordination for participants with very complex needs, multiple systems involved (child protection, justice, mental health), or significant risk factors. Requires specialized expertise and frequent support.
How It Works
Step 1: Initial Meeting
We get to know you, your goals, your NDIS plan, your current supports, and what challenges or gaps you’re experiencing.
Step 2: Action Plan Development
We develop a coordination plan outlining priorities, provider connections needed, capacity-building goals, and next steps.
Step 3: Provider Research & Connection
We research quality providers in your area, introduce you to options, and support you to make informed choices about who you work with.
Step 4: Ongoing Check-Ins & Support
We regularly review your supports, address any issues, help you track progress, and prepare for plan reviews. Contact frequency depends on your needs (weekly, fortnightly, monthly, or as-needed).
Step 5: Transition to Self-Management (If Appropriate)
If your goal is to eventually self-coordinate, we gradually build your skills and confidence until you’re ready to manage independently.
